    Holistic Dental Associates ("we," "us," or "our") is committed to protecting your privacy. In no event will we use or disclose your Part 2 Program record in any civil, criminal, administrative, or legislative proceedings against you, unless authorized by your consent or a qualifying court order.

    Complaints

    If you believe your privacy rights have been violated, you may file a complaint with our office or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
